Sports Physicals: You may pick up a sports physical form in the office or find a copy on the WIAA website. You may drop off a copy of the physical in the main office.
Summer Office Hours: June 1-August 14: Monday-Friday 7:30am-3pm
Physical examinations taken April 1 and thereafter are valid for the following two school years; physical examinations taken before April 1 are valid only for remainder of that school year and following school year.
